• Basic Life | One times earnings up to a maximum of $1,000,000
• Employee Supplemental Life | Choices of one to ten times annual earnings, to a maximum of $2,000,000
• Spouse Supplemental Life | You may purchase life insurance for your spouse in the amounts of $10,000; $25,000; $50,000; $100,000; $150,000 or $250,000.
  You must be enrolled in employee supplemental life in order to be eligible to enroll in spouse life. This means that employee life must be elected and approved for spouse life to be applicable.
• Child Supplemental Life | You may purchase life insurance for your child(ren) in the amounts of $5,000; $10,000; $25,000
• Guarantee Issue | Employee - Lesser of 3 times your annual salary up to $650,000; Spouse - $30,000; Child(ren) - $25,000
• Evidence of Insurability | Required to complete health questionnaire if you apply after your initial enrollment.

The definition of earnings includes your gross annual rate of pay including commissions, but excluding overtime and bonuses.

• Basic AD&D | One times earnings up to a maximum of $1,000,000
• Employee Voluntary AD&D | $25,000 to $2,000,000 coverage in increments of $25,000. The maximum amount of coverage is the lesser of 10 times your basic annual earnings of $2,000,000
• Spouse Voluntary AD&D | 100% of your coverage amount, not to exceed $1,000,000
• Child Voluntary AD&D | Cannot be greater than 100% of your coverage amount, not to exceed $300,000
